Services Connection Program
NCPHS has seven Service Coordinators placed in 16 San Francisco Housing Authority senior and disabled communities. This program focuses on residents who have been documented as isolated, economically disadvantaged, often newer monolingual immigrant populations, and without access to typical home and community-based services that can increase their independence and well-being.
The Service Coordinators perform outreach and direct social services introducing residents to services and benefits that could increase their functioning and abilities to remain safely in the place they call home. The Service Coordinators conduct assessments, provide case management, advocacy, and provide linkage and referral to other available services. Service Coordinators introduce educational programming on wellness and community resources and foster community building events.
